Targeted Case Management (TCM) Services
Targeted Case Management (TCM) is defined as those services which will assist the individual in gaining access to medical, social, educational and other needed services. Case Managers are required to conduct monthly on-site visits to assure quality of services and assure the individual's needs are being met. Case Managers are responsible for completing necessary paperwork to access needed funding and reviewing documentation of services being provided.
Targeted Case Management is a Medicaid funded service.
