The following are the definitions and descriptions of services that are offered in the CDDO of Southeast Kansas service area. Community Service Providers affiliate with the CDDO of SEK to provide as many of these services as possible. An individual may or may not be able to access a particular service depending on the availability, limitations of that service, or the funding sources available.
Licensed Services
Targeted Case Management (TCM) Services:
Targeted Case Management (TCM) is defined as those services which will assist the individual in gaining access to medical, social, educational and other needed services.Case Managers are required to conduct monthly on-site visits to assure quality of services and assure the individual's needs are being met. Case Managers are responsible for completing necessary paperwork to access needed funding and reviewing documentation of services being provided.
Targeted Case Management is a Medicaid funded service.
Day Supports are provided outside of the individual's home and include activities that are in accordance with the day activities indentified in the person-centered plan such as:
Employment activities, including pre-vocational activities which are aimed at preparing the individual for paid or unpaid employment such compliance, attendance, task competition, problem solving and safety.
Valued activities; those activities that the individual finds valuable.
Funding for Day Supports is through the HCBS I/DD waiver, if available.
(These services can be self-directed through Personal Care Services (PCS).)
Residential Supports are provided to individuals who do not live in a family home. This supports includes assisting with activities of daily living (ADL'S).
Funding for Residential Supports is through the HCBS I/DD waiver, if available.

Non-Licensed Services
Assistive Services include home modifications, van lifts, communication devices, and wheelchair repair.
Funding for Assistive Services is through the HCBS I/DD waiver, if available.
Financial Management Services (FMS):
These agencies provide billing and payroll services to individuals who self-direct their services. Individuals can self-direct the following services: Personal Care Services (PCS), Specialized Medical Care, Overnight Respite Care and Enhanced Care Services.
Funding for Financial Management Services is through the HCBS I/DD waiver, if available.
The primary purpose of Enhanced Care Services is to give overnight assistance to individuals who live with family. This service is limited to those who are unable to be left alone at night due to anticipated medical problems and the need for this support must be documented by individual's physician.
Funding for Enhanced Care Services is through the HCBS I/DD waiver, if available.

Overnight Respite Care is designed to provide relief for the individual's family member who serves as an unpaid primary caregiver.

Medical alert and other monitoring systems provide support to the beneficiary having a medical need that could become critical at any time.
The purpose of this service is to provide support to a consumer who has a medical need that could become critical at any time. The medical alert device is a small instrument carried or worn by the consumer which, by the push of a button, automatically dials the telephone of a predetermined responder who will answer the call for help.
Funding for Medical Alert services is through HCBS I/DD waiver, if available.
Personal Care Services (PCS) provide one on one support to individuals who live with their family or individuals that choose to self-direct their Day or Residential. These services include assisting with activities of daily living (ADL’s), community activities and activities other than employment.
Funding for PCS is through the HCBS I/DD waiver, if available. These services may be agency directed or self-directed.
Specialized Medical Care provides long-term nursing support for medically fragile and technology dependent individuals. This service may only be provided by a registered nurse (RN), a licensed practical nurse (LPN) under the supervision of an RN, or another entity designated by Kansas Department of Aging and Disability Services (KDADS).
Funding for Specialized Medical Care is through the HCBS I/DD waiver.
Supported Employment is competitive work in an integrated setting with on-going support services. These supports assist the individual to sustain paid employment in the community and cannot be provided in a sheltered workshop setting.
Funding for Supported Employment is through the HCBS I/DD waiver, if available.
Wellness Monitoring provides a registered nurse to evaluate the level of wellness of an individual once every 60 days.
Funding for Wellness Monitoring is through the HCBS I/DD waiver, if available.
